It is Up, Down, Up+Square button (Ps2 version). Works with characters with Axes as well.

Off the top of my head, these are the characters that can use their weapons in fatalities:
Scorpion, Sub-Zero, Cyrax, Dairou, Jarek, Fujin, Mavado, Kano, Kenshi, Shang Tsung, Mileena, Ashrah, Li Mei, Kira, and Sareena. I think that's all of them.
Just a note, although Mavado can use his hookswords in a fatality, Kabal cannot.
Here's a cool weapon based Fatality that works with every one of those characters:
1. Up, Up, Triangle
2. Up, Down, Up, Square
3. Down, Back, Square (Knees transition)
4. Forward, Forward, Square
5. Down, Down, Square
6. Forward, Forward, Square
7. Down, Back, Square (Standing Transition)
8. Forward, Forward, Triangle
9. Back, Back, Triangle (NOTE, See bottom)
10. Back, Back, Square
11. Down, Back, Square (Knees Transition)
12. Forward, Forward, Triangle
13. Up, Up, Square
14. Down, Back, Square (Standing Transition)
15. Back, Back, Circle
NOTE: When using Back, Back, Triangle, with a weapon, most weapons impale, but Kano, Mavado, Mileena, and Kira do not do it that way. For their impale, press Up, Down, Up, Triangle.
Here's a written summary of my fatality, using Scorpion as an example:
Scorpion rips off the opponent's arm, draws his weapon, then knocks them to their knees. He hits them with a right punch, a roundhouse kick, and using momentum from the roundhouse, hits them with a harder right punch. Scorpion then knocks the opponent to his feet and chops off his Left arm, then quickly impales and roundhouse kicks him. Scorpion knocks him to his knees again, then knees him in the face and hits him with a left punch and knocks him back up to his feet. At this point I usually snap their neck, but you can also rip their head off (Up, Up, Circle), crush their head, (Forward, Forward, Circle), or rip off their torso (Forward, Back, Circle).
Do not use Down, Down, Circle for this fatality. Using the spine rip turns them around causing the impaled weapon to sink in the ground when they fall, due to poor collision effects, and it detracts from the realism.

Kira was the last one I figured out that uses a weapon during a Fatality. Some of the characters are unique, Kano, Mavado, Mileena, and Kira can impale the head of their opponents with (Up, up X), they can also stab the head and stomach (Back, Back, X and Down, Down, X)
Some characters, Cyrax for example, can knock the opponent face down, then impale their head and pull it off with the head still on the sword (Forward, Forward, Circle, while they are face down).
Characters like Shang Tsung, or Sareena, can put the opponent in the "knees away transition", Where the opponent is on their knees and facing away, and then stab impale them in the back (Back, Back, Square), however this drains the Fatality meter, so if you do this, it's best to do that move as one of your first in the sequence.
Also those characters like Shang Tsung and Sareena can knock the opponent face up, then, like a one-handed golf swing, chop off their head and send it flying (Up, Up, Circle while they are face up).
So much info... I think I'll make a Fatality FAQ and request it get stickied. I'll do that now.
