No, Red Dragon wasn't that active back in MK3. It was Shao Kahn's extermination squads who horribly disfigured his face.

Does it really matter why? Most of SM is non-canon anyway.
This is from Wikipedia: "Surviving an attack from Kahn's extermination squads, horribly mutilated, he was kept alive by artificial respirators" which probably explains why his mask looks like that.
Besides Mavado couldn't have disfugured Kabal's face cuz there's no real "mention" of Mavado till before MK4 (which he wasn't in and he wasn't mentioned by name, FYI for all you d*cks who wanna argue 'bout that). And apparently Mavado kicked Kabal's ass and stole his Hookswords to use in Deadly Alliance
